Community Programs

 We make a difference in the community with our five key programs.  Learn about each of our community programs below or if you are ready to help our organization in our mission, we’d love to have you as a volunteer.

Personal Emergency Response

Our Lifeline Program provides personal emergency alert systems that can be placed in the home and can allow an individual to connect to resources for assistance.  These lifelines are availbale for private pay, or subsidized by our foundation or paid in full if needed.  These devices save lives everyday!

Sexual Assault Nurse Examiner

Our Sexual Assault Nurse Examiner (S.A.N.E) program helps connect victims and survivors of sexual assault and domestic violence to the resources in our community.  Our foundations supports the education of the S.A.N.E. nurses who are specialize in collecting evidence forensically.

Medical Transportation Program

Need a ride?  We can help.  Get a ride to and from the Aurora facilities in Hartford / Slinger, the Slinger dialysis center, Germantown Cancer treatment center, adult day care in Hartford, and to / from the Albrecht Free Clinic in West Bend.

Adult Day Service

Caring for an elderly parent or spouse is a daunting responsibility. Physical limitations, sickness and the deterioration of mental faculties, each present their own challenges. Combined, they become overwhelming. Know that you are not alone.

 

Our Adult Day Services program is designed to promote independence in the elderly and deter the need for nursing home placement. It is a community-based center that meets the health and social needs of older adults and their families.

Fitness Program

Enjoy the opportunity to heal with professional trainers who are truly watching over you in our fitness program. The Medical Center Foundation of Hartford provides funding for community members to continue their rehabilitations needs at the Aurora Medical Center of Washington County Fitness Center.

Every participant’s blood pressure will be taken before and after exercising. Oxygen levels are monitored throughout the workout. Personal trainers will be made available for all participants. A thorough evaluation is assessed prior to a personal program being set up.
