Adult Day Services

Caring for an elderly parent or spouse is a daunting responsibility. Physical limitations, sickness and the deterioration of mental faculties, each present their own challenges. Combined, they become overwhelming. Know that you are not alone.

How does this program work?

Our Adult Day Services program is designed to promote independence in the elderly and deter the need for nursing home placement. It is a community-based center that meets the health and social needs of older adults and their families. This program runs Monday – Friday 7:00 am – 5:00 pm. Threshold, Inc and the Medical Center Foundation of Hartford are in a partnership. This partnership is terrific and continues to serve the community very well. Your loved one receives wonderful care and love from their professionally trained staff.  Complementary visits are available call today for your scheduled visit 262-808-2946.
How much does it cost? 

As a program supported by Medical Center Foundation of Hartford, the Adult Day Services participants are charged only a portion of the program’s cost. In 2020, the per-day rate is based on level of care required. It is by far less than the total $185 plus or more per-person daily rate at a nursing home facility. State funding and subsidizing programs are available through the county and through the Medical Center Foundation of Hartford. This assistance makes this service more affordable and of course, your donations help us help others.

Threshold, Inc. will continue to provide this care until April 1, 2021.  The Medical Center Foundation of Hartford is actively looking for a new care provider.  This program is very important and will continue to be a service offered in our community. Available on an as-needed basis, our Adult Day Services program provides flexibility and scheduling options to meet individual needs. Our companionship and personal care creates an environment that feels like home. Services include:

  • Fireplace, resting areas, computer access for members
